Forms needed to apply for a Michigan Medical Marijuana Registry identification card.

 

This first form is the Qualifying Patient Certificate. This form is for your physician to complete describing your debilitating medical condition and his authorization for you to include medical marijuana in your treatment.

 

The next form you will need to have your physician complete is the Medical History Form. It's your physician's assessment of the your chronic pain and current medical condition.

This form is the Doctor's Opinion Memo and Authenticating Affidavit to support your defense. This is an affidavit that your physician must complete for you to be able to claim Affirmative Defense.

 

The Patients Affidavit in Support of Motion form is for patients to complete. It is your affidavit that you are using the Affirmative Defense and requesting that the case against you be dismissed.
