Current Change Form?

I have a friend who just applied for his card...He didnt name a caregiver and still has his plant rights...I went to the LARA site and looked for the form that he could use to name me as his caregiver but the form looked too similar to the old form that had the redundant questions on it....Does anyone have a link to the correct form? HJ

The current form is MMP 3502 (Rev. 4/13).  Unless you had old forms laying around and got them mixed up, you're good.

I've always printed fresh forms from the website. Make sure to "refresh" each time you visit the site. Recently the forms were

accepted with no issue.

Glad to hear I used the right one. I will admit when I printed it out and looked it over I thought, I swear they simplified this whole thing. Guess it was just the patient application that was simplified.

Correct I do not believe they revised the change form.
