So I just sent my cert in but my id and my patients id show the old address on the front and a sticker from the sos showing the new address on the back. Think this will mess things up?

nope they go by what you put on the form..

No problems, that is why they want you to copy both sides of  your I.D

yes it is in the package you get from your renewell or the first time you get your cert, it tells you what to send, try to send your i.d w/o both sides copied and than you can tell me what happens!

I looked at the form online just now....http://www.michigan.gov/lara/0,4601,7-154-72600_72603_51869---,00.html

 

 

says "Copy of Valid Photo ID (Michigan Driver’s license, Michigan ID card, or other acceptable form of ID)  The copy of the photo ID must be clear and legible.  If you submit a copy of a photo ID that is not a Michigan driver’s license or Michigan ID card, you must also submit a copy of your Michigan voter’s registration card as proof of residency."

 

do you have a new form with "both sides" mentioned?

Ive sent in with out both sides of DL for a long time...never had an issue...

I would think that the only feasible reason to copy both front and back of ID/ DL is if a person has moved and the address

on the back is different from that on the front and would like their card/ cards to be mailed to the current address (on back).
