Couple Questions Regarding Application Paperwork

Greetings all. Been lurking for a while, but finally have a couple of questions. So I first posted in the 'say hello & introduce yourself' section, and now I'm here to hopefully get a couple things cleared up.

 

My regular doc surprised me by telling me to bring in the paperwork and he'll sign off on it so that I can get my card. So I printed out the paperwork, filled out the application form, and have a question regarding the "Physician Cerification Form", as well as a question about mailing it in.

 

1) on the physician cert form, should I take it to my doc completely blank? Should I fill out section b, which is just my name & DOB? Part of me thinks I should just leave it blank so he can fill out the entire page himself, but part of me wants to fill out that section just to make sure there are no mistakes. Doesn't seem like a big deal, but it also seems like I've read of people being denied due to the smallest thing like a date being incorrect. So I just want to do whatever I can to make sure everything is ok.

 

2) it seems like I've read of a number of people sending in their paperwork via registered or certified mail. I don't see anywhere specifying the need to do this, so I'm curious as to why some people do it. I understand what that type of mail service does, so is it just for peace of mind knowing that it indeed was delivered to the correct place on a certain date?

 

Thanks for any help anyone can provide with clearing this up for me. I'm just so happy to finally be at this point of being able to get my card!

1) LEAVE IT BLANK, let the MD complete it

2) I use certified mail with return receipt so that I know right away the day my letter arrives.

I also use it for this because of the sensitive timing in all of this, and I know that that type of mail is handled more carefully

than a licked stamp piece. I count on it, maybe a waste dunno, definitely a piece of mind.

Thanks for the info and stressing the point of leaving it blank. That's pretty much what I was thinking, but a little part of me wanted to fill out that section with my name/DOB, just to be certain there was no mistake.

 

 

 

make sure when your dr fills out that part (he has to fill it out by law/rule) that he doesnt miss any spelling or DOB or anything.

 

Yeah, that's the part that I was worried about. But I'll just have to make sure it's all correct before leaving the office. I just know that sometimes I've got other things on my mind and I've had to return to his office after the pharmacist realizes something was written incorrectly, or was even omitted, on a prescription.

 

 

A huge thanks to the both of you. That was the information I was looking for, and since I'm going to his office this evening I am very grateful for the quick replies.
